The show doesn't outright tell you. However there is strong evidence to suggest that it is Gus or one of his men. He did this for a couple of reasons. 1.) Gus has a rivalry with the cartel ever since *spoilers* his partner, Max, was murdered by Hector. Gus (or one of his men) called Hank in order to give him a chance to defend himself. This was Gus' way of almost sticking it to the cartel without being too obvious. If Gus ordered a hit out on the brothers himself, it's likely the cartel would know and declare war on Gus and his operation. 2.) This reason is obvious. Being Walt's brother in law, it would obviously affect Walt's ability to cook and relationship with Gus if Hank was killed.
